| 目的 本文通過對(duì)惠州某醫(yī)院門急診2015年6月到12月胺碘酮抗心律失常的使用情況進(jìn)行調(diào)查和分析，了解醫(yī)院門急診胺碘酮抗心律失常的應(yīng)用情況，為臨床安全、有效、合理地使用胺碘酮提供參考。方法 統(tǒng)計(jì)惠州某醫(yī)院2015年6月到12月份門急診含胺碘酮處方，臨床診斷為（含）心律失常的處方378份，對(duì)患者基本信息、用藥指征、用法用量、藥物聯(lián)合應(yīng)用進(jìn)行分析和討論。結(jié)果 在378份含胺碘酮的處方的臨床診斷中，心房顫動(dòng)患者最多，其次是心衰，而早搏、支氣管炎或哮喘患者、尿毒癥患者屬于不合理用藥。在胺碘酮用法用量中qd最多，其次是bid，最少的是qod，其中>60歲，qod、qd、bid多于其他年齡段；胺碘酮與其他西藥的聯(lián)合治療中，常與地高辛、β受體阻滯劑、華法林聯(lián)用，其中與地高辛聯(lián)用最多；胺碘酮與中成藥的聯(lián)合治療中，常與麝香保心丸、穩(wěn)心顆粒、復(fù)方丹參滴丸聯(lián)用，其中與麝香保心丸聯(lián)用最多。結(jié)論 胺碘酮抗心律失常用藥指征、用量、聯(lián)合用藥等方面基本合理。 |

| Objective The paper aims to have a good understanding of the antiarrhythmic application of amiodarone in the outpatient emergency department and provide a reference for clinically safe, effective, and reasonable use of amiodarone by investigating and analyzing the use of amiodarone in treatment of arrhythmia in the outpatient emergency department at a hospital in Huizhou. Method 378 prescriptions containing amiodarone and arrhythmia were collected from the outpatient emergency department at a hospital in Huizhou between June 2015 and December 2015 to analyze and discuss the patients’ basic information, medication indications, dosage and administration, and combined application of drugs. Result Among the patients with diseases for which the 378 prescriptions containing amiodarone were dispensed, ones with atrial fibrillation had the most prescriptions, followed by ones with heart failure. Nevertheless, the prescriptions for the patients with premature beat, bronchitis, asthma, or uremia were unreasonable. In term of dosage and administration of amiodarone, qd had the highest usage frequency, followed by bid, leaving qod with the lowest usage frequency. It was used for patients >60 years and qod, qd, and bid largely for patients in other age groups. Amiodarone was often combined with other Western drugs such as digoxin, β receptor blocking agent, and warfarin. Digoxin was the most commonly used one in such combined treatment. Amiodarone was often combined with the Chinese patent medicines such as Shexiang Baoxin Pill, Wenxin Keli, Compound Danshen Dripping Pills. Shexiang Baoxin Pill was the most commonly used one in such combined treatment. Conclusion Antiarrhythmic application of amiodarone is considered to be rational in medication indications, dosages, combined medication, etc. |
