各位師兄師姐，我拿到了新澤西州羅格斯大學博士后的OFFER后，系里要求的材料我都給他們回復了，過了幾天收到了大學人力資源處的郵件，請教大家， Congratulations on your Post Doctoral appointment in the Rutgers. Please be aware that your offer is conditional based upon satisfactory employment references and background checks and your pre-employment physical. A copy of job description form is attached for your reference. Please sign and return all attached documents to me except the Employee Health Questionnaire form. Detailed below are the next steps in our hiring process if you are interested in moving forward. 1) Please complete and return the attached Information Sheet, along with a signed copy of attached Job Description and confidentiality Statement. *** If faxing, please return to 732.932.0046. 2) Once received, you will receive a separate email from our background service vendor, GIS (look for an email from: BIG10@geinfo.com ).You will need to enter the requested information within 3 days, in order for us to begin running a background check. 3) You will need to visit our Employee Health Services office located at 170 Frelinghuysen Road in Piscataway for a pre-employment health screening prior to coming on board. The physical will require two visits: the first to administer a TB test and the second to have the TB test results read and to complete your physical. Please plan to visit the EOHSI Clinical Center 2 weeks after you have submitted your background check information toeService@geninfo.com. Please contact me after your initial visit to inform me of the time of your follow-up appointment. We will then schedule a time to meet on that same day to complete your E-verify process.
