Clinical study on anterior reconstruction plate internal fixation for middle clavicular fracture 【摘要】 目的：探討鎖骨中段骨折重建鋼板前置內(nèi)固定的效果及可操作性，為臨床提供依據(jù)。方法：回顧2010年7月至2012年2月鎖骨中段骨折使用重建鋼板前置內(nèi)固定患者23例作為研究組，男19例，女4例，年齡37-78歲，平均（48.5±7.2）歲。2010年9月至2012年4月重建鋼板上置內(nèi)固定治療鎖骨中段骨折患者19例作為對(duì)照組，男14例，女5例，年齡39～57歲，平均（46.3±5.3）歲。記錄兩組患者術(shù)中鎖骨內(nèi)段、外段螺釘數(shù)目、長(zhǎng)度，以及手術(shù)暴露時(shí)間、出血量，進(jìn)行統(tǒng)計(jì)學(xué)分析。研究組患者定期拍X片，并記錄12周時(shí)骨折位置、疼痛情況及16周時(shí)肩關(guān)節(jié)運(yùn)動(dòng)功能恢復(fù)情況，并采用侯春林等的療效評(píng)價(jià)標(biāo)準(zhǔn)進(jìn)行評(píng)價(jià)。結(jié)果：前置重建鋼板組23例患者術(shù)后切口均一期愈合，無(wú)感染。12周時(shí)骨折均愈合。16周隨訪結(jié)果，優(yōu)22例，良1例。兩組骨折外側(cè)段螺釘長(zhǎng)度比較，u=63.21>1.64,p<0.05,內(nèi)側(cè)段螺釘長(zhǎng)度比較，u=5.46>1.64,p<0.05，前置重建鋼板組內(nèi)、外段螺釘長(zhǎng)度顯著大于相應(yīng)的上置重建鋼板組內(nèi)、外段螺釘長(zhǎng)度。V=40時(shí),兩組術(shù)中出血量比較，t=0.489<1.684,p>0.05,兩組手術(shù)暴露時(shí)間比較，t=0.912<1.684,p>0.05，兩組在術(shù)中出血量、手術(shù)暴露時(shí)間差別無(wú)顯著性意義。結(jié)論：鎖骨中段骨折重建鋼板前置內(nèi)固定比上置更穩(wěn)定，療效可靠，尤其對(duì)于中老年可疑骨質(zhì)疏松者。 |

自己動(dòng)手操作了一下，請(qǐng)教大蝦指點(diǎn)修正。多謝。 ABSTRACT Objective: To investigate the maneuverability and effect of treatment with anterior reconstruction plate internal fixation for middle clavicular fracture and to supply the base for clinical use. Methods: From July 2010 to December 2012 ,23 patients were treated with anterior reconstruction plate internal fixation for middle clavicular fracture (treatment group),including 19 males and 4 females with a mean age of（48.5±7.2）years (ranged, 37 to 78 years). From September 2010 to April 2012 ,19 patients were treated with upper reconstruction plate internal fixation for middle clavicular fracture (control group),including 14 males and 5 females with a mean age of（46.3±5.3）years (ranged, 39 to 57 years). The Index sunch as the screw number and length through the acromion and sternum segment ,blood loss and operative exposure time during operation were observed for the two groups, and for the treatment group, pains, fracture site and shoulder function were observed at 12th weeks after operation and evaluated at 16th weeks. Results: The treatment group had a good HOU Chun-lin’s clavical score （22 excellent patients and 1 good）at 16th weeks, and incision healing in time, no infection, fracture union at 12th weeks.The acromion segment screw length of the treatment group was significantly larger than that of the control group(p<0.05),and the Sternum segment screw length of the treatment group was significantly larger than that of the control group(p<0.05),;while there was no significantly difference Between the blood loss and operative exposure time(p>0.05).Conclusion: The anterior reconstruction plate internal fixation for middle clavicular fracture has better stability and more reliable effect than the upper reconstruction plate internal fixation, especially for the middle and old age who has the suspected risk factors of osteoporosis.
