求英語專業(yè)的同仁，翻譯一份問卷，論文急用！

由于論文需要，急需翻譯一份外文問卷，本人英語超爛，希望英語底子好的，幫忙翻譯一份問卷，39個題目，不甚感激！！論文急用！ Please circle the number that corresponds to the degree to which you agree with each statement. 1.
