Indeed, Cullion et al were able to show that a GSI dosing schedule with drug holidays largely avoided the gut toxicity while maintaining significant efficacy in mouse T-ALL models . It will be interesting and exciting to determine if strategies based on the same principle could expand the therapeutic window of selective targeting of DLL4/NOTCH1. The other remarkable observation from Ferrando’s group was that dexamethasone protected mice against GSI-induced gut toxicity by shifting differentiation away from goblet cell development |

| 事實(shí)上，Cullion等顯示γ-分泌酶抑制劑（GSI）與用藥假期相結(jié)合的療程在很大程度上避免了腸道毒性，同時(shí)在小鼠急性T淋巴細(xì)胞白血病(T-ALL)模型上有顯著的療效。這將令人感興趣也令人興奮去確定基于相同原理的策略能否擴(kuò)大靶向DLL4/NOTCH1的治療窗口。Ferrando小組的其他重要觀察是地塞米松通過去掉小鼠杯狀細(xì)胞發(fā)育的分化而避免GSI誘導(dǎo)的腸道毒性。 |
