早期HBO治療重型顱腦外傷療效及抗感染作用的臨床研究 [摘要]目的：探討早期高壓氧(HBO)綜合治療對(duì)重型顱腦外傷患者腦功能康復(fù)的臨床療效及抗感染作用。方法：將重型顱腦外傷患者隨機(jī)分成2組，治療組68例，神經(jīng)外科治療、綜合治療的基礎(chǔ)上行早期高壓氧治療；同期常規(guī)治療組68例，行神經(jīng)外科常規(guī)治療、綜合治療。比較2組患者治療前后格拉斯哥昏迷量表(GCS)評(píng)分變化情況，并于治療后3、6、9個(gè)月進(jìn)行格拉斯哥預(yù)后量表(GOS)評(píng)定；同時(shí)分別比較兩組治療前后感染發(fā)生率及傷后1w、2w、3w、4w外周血白細(xì)胞變化情況。結(jié)果：1. 治療前兩組GCS差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05)，治療后兩組GCS差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。2、治療前兩組GOS評(píng)定差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05)，治療后兩組GOS評(píng)定差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。3、治療組感染發(fā)生率較低，特別是呼吸道、皮膚感染發(fā)生率差異有統(tǒng)計(jì)學(xué)意義（P<0.05）；傷后1w兩組患者WBC較正常值高，但差異無(wú)統(tǒng)計(jì)學(xué)意義（P>0.05）；傷后2、3、4w，治療組WBC較對(duì)照組低，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）；傷后3、4w治療組WBC恢復(fù)正常值范圍。結(jié)論：重型顱腦損傷患者在神經(jīng)外科常規(guī)治療、綜合治療的基礎(chǔ)上結(jié)合早期HBO治療能提高恢復(fù)良好率，降低死亡率和感染率，明顯改善患者的生存質(zhì)量。 [關(guān)鍵詞] 高壓氧；顱腦損傷；GCS；GOS ；WBC；感染率 |

Studies of the clinical efficacy of early HBO treatment in severe traumatic brain injury and its' effects in anti-infection Abstract Objective To explore clinical efficacy and anti-infection effects of hyperbaric oxygen (HBO) for comprehensive treatment of brain function rehabilitation in patients with severe traumatic brain injury . Methods Patients with severe traumatic brain injury were randomly divided into two groups, the (HBO) treatment group of 68 patients with neurosurgery treatment, comprehensive treatment and early hyperbaric oxygen therapy; the conventional treatment group of 68 patients underwent conventional neurosurgery treatment and comprehensive treatment.Two groups were compared with Glasgow Coma Scale (GCS) score changes before and after treatment , and Glasgow Outcome Scale (GOS) assessment at 3,6,9 months after treatment . The incidence of wound infection and leukocytes changes in peripheral blood after 1w, 2w, 3w, 4w of the injury of the two groups were compared before and after treatment. Results: 1 Before treatment GCS showed no significant difference (P> 0.05), GCS difference between the two groups after treatment was statistically significant (P <0.05).2, GOS assessment before treatment groups showed no significant difference (P> 0.05).After treatment, both groups were statistically significant differences by GOS assessment (P <0.05).3, lower infection in the treatment group, especially respiratory. Skin infection rate was significantly (P <0.05) between the (HBO) treatment group and the conventional treatment group ; patients WBC after injury of 1w were higher than normal, but the difference was not statistically significant (P> 0.05); after injury of 2,3,4 w, WBC of treatment group compared with the control group, the difference was statistically significant (P <0.05); WBC of treatment group after injury of 3,4 w WBC became normal . Conclusion Patients with severe brain injury in conventional neurosurgery treatment, combined with comprehensive treatment and early HBO therapy can improve recovery rate, lower mortality and infection rates, and significantly improve the quality of life of patients.
